The South Beach Diet is a three-phase low-carb dietary approach developed by Miami cardiologist Arthur Agatston in the late 1990s. Unlike Atkins, which focuses purely on carbohydrate restriction, South Beach also distinguishes good fats from bad: it encourages unsaturated fats (olive oil, avocado, nuts) while limiting saturated and trans fats.

The protocol: Phase 1 (2 weeks, strict) eliminates all refined carbohydrates, sugar, fruit and alcohol — rapid initial weight loss. Phase 2 gradually reintroduces whole grains, legumes and most fruit — continued gradual loss. Phase 3 is the lifelong maintenance phase with all food groups included in appropriate proportions.

Arthur Agatston developed the South Beach Diet in the late 1990s while working as a preventive cardiologist in Miami, initially to help his patients improve cardiovascular markers. Frustrated with the low-fat dietary advice prevailing at the time, which he felt wasn’t working for his patients, he began developing a protocol based on glycaemic index research and the emerging science distinguishing healthy from unhealthy fats.

He published The South Beach Diet in 2003. The book became one of the bestselling diet books in American history — over 23 million copies sold worldwide in multiple languages. The “South Beach” name evoked the glamorous, health-conscious Miami Beach aesthetic and resonated with readers in a way that a purely clinical title wouldn’t have.

Agatston is also known for developing the Agatston calcium score — a cardiac CT measurement for coronary artery calcification that has become a standard tool in cardiovascular risk assessment. This clinical credibility differentiated him from popular diet-book authors without medical backgrounds and gave the South Beach Diet a degree of institutional credibility unusual in the diet book market.
